The Role
Job description (Job Specific Information)
The positions entail working with people with very mild learning disabilities, autism, Asperger’s and or other related conditions to access community services and social activities. Such activities normally involve supporting an individual to go shopping, library, cinema, support to the doctors, going out for trips among others. Other support provided would be travel training, project planning, support in class or school.
However, the main aspect of support would require someone who is able to remain calm when the person being supported becomes anxious or unsettled which may make then unsafe to themselves or those around them.
We provide flexible work such that you can work as little as 3 hours a week to 40+, on weekdays, weekends and evenings. By knowing your availability we would be able to see what work would suit.
You must hold a licence which allows you to drive in the UK, clean for more than one year, together with your own transport (can be a motorcycle or moped) with business insurance. You must be willing and able to travel across the area, supporting young people from their own homes. Reasonable mileage is reimbursed.
Notes:
- Most of the support we provide is Monday- Sunday during the hours of 8am-10pm, but some support is required in the evenings.
- Some positions do not require a driver. Please enquire.
- Hours can be combined to have a “long day”.
- Shifts: long, short, afternoons, evenings, weekdays, Saturdays Positions will require a Disclosure Barring Service (DBS) certificate and references before commencing.

Purpose Of Post
To work as part of a team providing high quality care and support to Service Users one to one or as part of a group to take part in daily activities.
Main Responsibilities:
To assist Service Users with their activities within their community, to support with their budgets, to create new opportunities for Service User to take part in activities within their own community.
Responsibilities to the people we support and your responsibilities to yourself:
- To ensure that people we support are treated with respect, dignity and equality.
- To be personally accountable for the standard of your practice.
- To safeguard and promote the welfare of the people we support.
- To complete daily budget and other documents.
- To maintain the confidentiality of information given to you either written or verbal
- To be responsible for your personal development and undertake training as required.
- To participate in regular supervision sessions and annual appraisals.
- To undertake responsibilities with regard to the Health and Safety at Work Act.
- To set up activities and tidy up. If the building needs to be lock up this must be done and key return to the line manager.
- Maintain professional boundaries at all time, this include communication between staff and the service users.
- Please note: You are responsible for your own belongings and GFI takes no responsibilities of loss items or your own personal money. See contract for more information.
- To be responsible for a named service user, this may include picking them up from their own home and returning the service user to their home. You must stay with your service user until handed over to parents/carer or authorized centres i.e. respites, and at no time leave the service user unattended.
- To work with service users 1:1 or in a small group basis to participate in activities of their own choice and following parents or carers advice on activities in the venue and the community.
- To keep daily records including activities diary, behaviour and any other form of records the line manger needs.
- To plan activities with the learner and within their learners budget
- To work with medical and/or social and health care professionals on any specialist interventions/ strategies as agreed with the learners, their family, carers, etc.
- To support and assist each person to maintain a high standard of personal care including, intimate care, hygiene and physical wellbeing.
- To communicate with families, representatives and outside agencies in a professional and courteous manner.
- To attend yearly learner reviews and other meetings when required.
- To bring to the attention of a PDP Manager of any matter for concern over the wellbeing, safety or safeguarding of a person we support.
- Where required and authorised, to drive the vehicles used by our learners and to participate in ensuring the upkeep and maintenance of any such vehicles. (if you using your own, the management request to see original of business insurance for your vehicle)

The Role
Job description
We are looking for an enthusiastic and motivated person to be on the management team of a Personalised Day Provision (PDP) for 15 adults with a range of learning disabilities including mild to moderate learning disabilities, physical health needs as well as high functioning autism and Asperger’s.
The role of the Service Manager is to provide leadership, development and direction, ensuring that the team provides the people we support within the service with the support they need to lead a good life and achieve the things that are important to them and their families.
There will be involvement in all areas of people management, setting expectations and monitoring performance, providing One to One meetings and Annual Reviews to ensure staff are demonstrating the skills and attitudes needed, maintaining their motivation and development by working alongside staff, role modelling good practice, coaching and mentoring.
1) Build great relationships with the people we support and the people important in their lives (families, other staff, and external professionals). Know their skills, interests, qualities, strengths, challenges, ways of communicating and aspirations and ensure the individualized service they receive plays to their strengths; helps them tackle their challenges and achieve their goals.
2) Ensure people are supported in everyday tasks in a respectful way that meets their individual needs and preferences and enables them to do as much as possible themselves; to give them as much choice, control and involvement as possible. This would include assistance with communicating their needs, working out their money, getting in touch with friends, taking part in classes, taking them to their appointments, or linking with the community.
3) Key work with the Individual(s) to develop their support plan through the referral, assessment and review stages of person-centred planning. Understand their lifestyle choices, circles of support, what is important to and for them, and supporting them to have more choice and control in their life.
4) To develop and implement individual care plans; ensuring all plans and records relating to people we support are accurate and up to date using respectful language which allows them to see what progress is being made against goals; track areas of concern (e.g. health) and take action as agreed.
5) Support people to have their own voice heard wherever possible and where needed to speak up for them and their interests e.g. when out and about, with other professionals, within the team. Consistently look to find ways to ensure people have as much power and control over their lives.
6) Work in a consistent and positive way with other team members to ensure people get a great service from the whole team at any time. Ensure all organisation policies and specific agreed ways of working in the service are followed e.g. that specific support plans are being followed up, processes are being adhered to, that money is managed effectively, that health and safety practices are carried out.
7) Supervise, coach and train inexperienced and junior colleagues in person centred support practices and your specialist area of expertise, giving and acting on received feedback on what is going well and what is not going well.
8) Ensure effective day to day management of colleagues inclusive of activity delegation, quality assurance, performance management and recruitment and selection as appropriate to the role.
9) Develop and maintain links with professional bodies that support your area of expertise such as Autism, Intensive Interaction, community connecting, employment and training implementing changes in practice within the organisation.
10) To observe changes within the individual’s circumstances and respond accordingly as well as to undertake personal care tasks in a sensitive manner to meet the needs of the people we support.
11) To use information technology (including a computer) to undertake a range of administrative tasks and to follow procedural guidelines and complete appropriate documentation in required formats and to compile detailed written reports and be able to communicate clearly to people we support, employees and other professionals all the while understanding the nature of confidentiality.
12) Ensure the team can balance the needs for safety and risk taking, so that individuals supported do get the opportunity to experience new things in their lives and achieve their goals and plans.
13) Ensure that the staffing rota meets the needs of the people being supported and that as much as possible staff are well matched to specific activities and individuals.
14) Ensure that all the key quality indicators and safety measures are met, audits are carried out as required, resources are managed efficiently, and concerns are followed up swiftly.
15) Work in partnership with the Manager to ensure that the finances of the team and especially the finances of individuals we support are being effectively managed and the service complies with standards and requirements of the organisation and regulators.
16) Work in partnership with the Manager to produce, monitor and update operational documents, such as risk assessment, policies, service user files and any other related task.
